Dr. Alex Kim is a hospital administrator with special interests in patient safety and healthcare quality. He graduated from the Science University of Malaysia and completed his MBA in hospital administration. He was conferred as a fellow by the International Society of Quality in Health Care (ISQua) since 2018. He served as the hospital director of Kanowit Hospital and he was the head of quality directorate at Sarawak General Hospital. Last year, he received a full scholarship from the Japanese government and is currently pursuing his second master’s degree (Master of Healthcare Administration) at Nagoya University under the Young Leaders’ Program.

Dr Ariffin Marzuki bin Mokhtar is a National Specialist Registered Anesthesiologist that graduated from the Universiti Kebangsaan Malaysia (UKM) in 2002 and is currently lecturing Medicine at the School of Medical Sciences, Universiti Sains Malaysia. His clinical speciality is Cardiothoracic Anesthesia and Critical Care with an interest in Pain Management, Extra-corporeal Membrane Oxygenation and Mechanical Circulatory Support. Dr Ariffin is a Certified IT Architect -Associate: Business Architecture (CITA-A, IASA), COBIT 5 (ISACA) and TOGAF 9.1 (Open Group) Certified. He has served as the Deputy Director (Operations) and is currently the Hospital Management System Co-ordinator for Hospital USM, where he coordinates the operation and the Information, Communications and Technology (ICT) initiatives of the Hospital USM by managing multiple stakeholders and providers to ensure value-delivery in the ICT initiatives for the hospital in terms of patient and student centricity, staff competency building and developing a resilient knowledge management system. His current interests are the use of technology as an enabling tool to deliver high quality and safe care to patients including competency building of healthcare providers in using technology tools, modelling clinical care, development of clinical pathways, performance measures, healthcare organization transformation governance and management, healthcare business architecture, activity-based costing and financial management.

Dr YK Chan is currently an honorary professor in the Department of Anesthesiology, Faculty of Medicine, University of Malaya. Having spent a lifetime training medical students and postgraduate students in the science and art of medicine, she now involves herself mainly in medicolegal work and in training others in the field. Whilst both have given her joy, she finds most happiness in helping others find peace through showing how this can be achieved in a high-pressure environment. Life has been harsh for her but with determination and perseverance, she managed to overcome many of life’s limitation through help from others. She now helps others achieve wellbeing by sharing generously of herself, her time, energy, and her expertise whenever anybody is in need.

Dr. Arnal is anaesthesiologist, Head of the Surgical Process, at the Hospital Universitario Fundacion Alcorcon, Spain, and is the European Society of Anaesthesiology and Intensive Care (ESAIC) Patient Safety and Quality Committee past Chair. Dr. Arnal is cofounder and past president of the Spanish Anaesthesia Incident Reporting System (SENSAR), and Vice-president of the Quality, Patient Safety and Ethics of the Spanish Society of Anaesthesiology and Reanimation (SEDAR).
His publications and scientific interests focus on Patient Safety and is Co-editor of the Spanish Anaesthesia and Critical Patient Crisis Manual. As clinical simulator instructor participates as faculty in IDEhA Simulation Centre (Alcorcon), SENSAR and ESAIC patient safety continuous education programs.

Daniel J. Cole earned his Doctor of Medicine degree from Loma Linda University, where he also completed his residency in anesthesiology. He completed a Neuroscience Research fellowship at the University of California at San Diego.
Dr. Cole currently serves as the Vice Chair for Professional and Business Development for the Department of Anesthesiology and Perioperative Medicine at the University of California, Los Angeles. He previously served as Chair at Mayo Clinic in Arizona and as Vice Dean for Continuous Professional Development, Mayo Clinic.
A neuroanesthesiologist by training, he has published extensively, with more than 350 original manuscripts, chapters, abstracts, and editorials to his credit. Over the course of his career, he has served as a reviewer for more than 25 medical journals. Dr. Cole is also a distinguished lecturer with over 300 invited presentations on a broad range of topics.
Currently, Dr. Cole serves as the Executive Director for Professional Affairs for the American Board of Anesthesiology, and serves on the Board of Directors of the American Board of Medical Specialties. Dr. Cole is a past president of the American Society of Anesthesiologists; and serves as President of the Anesthesia Patient Safety Foundation.

Dr. Della Lin is an anesthesiologist with a leadership career spanning over 3 decades. She has a passion for patient safety and was an inaugural patient safety leadership fellow with the National Patient Safety Foundation/American Hospital Association in 2002. She continues her safety passion as APSF board member, as a committee member and reviewer with Closed Claims at ASA’s AQI, and as Senior Fellow in Patient Safety Leadership with the Estes Park Institute.
Dr. Lin serves as faculty for the Institute for Health Care Improvement (IHI) and holds an Associate Faculty appointment with Ariadne Labs–a joint center for health systems innovation at Brigham and Women’s Hospital and the Harvard T.H. Chan School of Public Health. While partnering as Visiting Faculty with the Johns Hopkins Armstrong Institute for Patient Safety and Quality, Dr. Lin led cross-continuum community collaboratives, including leading the Hawaii CUSP-STOP BSI and Hawaii Surgical Unit Safety Program efforts where statewide hospital-acquired infections were reduced by >80% and >65% respectively, and being part of the national team implementing the AHRQ grant around Enhanced Recovery after Surgery. Dr. Lin has held numerous positions within hospital/departmental leadership including Chief, Department of Anesthesiology, Board Quality Committees, MEC and Peer Review.

Fred E. Shapiro, D.O., FASA is an Associate Professor of Anesthesia, Harvard Medical School, Massachusetts Eye and Ear (MEE) Boston, MA. He has extensive research publication experience and a reputation nationally and internationally in the field of patient safety in anesthesia and surgery performed outside the operating room.
He created first Harvard Medical School Office- based anesthesia CME Course, “The Manual of Office Based Anesthesia Procedures”, which led national OBA curriculum for the Society for Ambulatory Anesthesia. He wrote the seminal ASA CME article on Office Based Anesthesia: Safety and Outcomes.
He participated in The MA Board of Medicine task force to define qualifications for practitioners supervising office-based procedures and surgery, recently revised the Massachusetts Medical Society (MMS) Office Based Surgery Guidelines and act as a resource to ASA governance regarding similar statewide national legislative changes.

Elizabeth Rebello, MD, FASA, CPPS, CMQ is a practicing anesthesiologist and Professor of Anesthesiology and Perioperative Medicine at the University of Texas MD Anderson Cancer Center. Within her institution, she also serves as the Executive Director for Anesthesiology, Critical Care and Pain Medicine for the Cancer Network A pharmacist by training, she has been involved in quality and patient safety efforts in the perioperative environment and Diversity, Equity, and Inclusion efforts for over fifteen years.
She serves as the Chair of the American Society of Anesthesiologists’ (ASA) Quality, Management and Departmental Administration Committee and led the Medication Safety Workgroup to standardize drug concentrations. She serves on the United States Pharmacopeia Expert Compounding Committee, US Food and Drug Administration Pharmacy Compounding Advisory Committee, and chairs the APSF Patient Safety Advisory Group on Medication Safety.

Dr Hasmizy Muhammad is a Consultant Cardiothoracic Anaesthesiologist and Head of Department, Department of Anaesthesiology and Intensive Care, Sarawak Heart Centre, Malaysia. He received a medical degree in 1998 and a Master of Medicine (Anaesthesiology) in 2006 from the University of Science Malaysia. He was trained in cardiac anaesthesia and perfusion at Sarawak General Hospital in 2008 and Konkuk University Medical Center, Seoul, South Korea, in 2011. He obtained the Graduate Diploma in Clinical Ultrasound from the University of Melbourne in 2016, the Master in Business Administration from University Tun Razak in 2022, and gained experience in ECMO at the University Hospital Regensburg (Germany) and paediatric cardiac anaesthesia at the Royal Brompton Hospital (UK).
Dr. Hasmizy Muhammad is the chairman of the Sarawak Perfusion Society, vice president of the Malaysian Cardiothoracic Anaesthesiology and Perfusion Society (MASCAP), a council member of the College of Anaesthesiologists, Academy of Medicine of Malaysia, and an executive councillor of the Malaysian Society of Anaesthesiologists. He is an examiner for the degree of Master of Medicine (Anaesthesiology) conducted by the Board of Examination of Anaesthesiology & Intensive Care in Malaysia. Dr. Hasmizy Muhammad is actively involved in teaching and training in the fields of anaesthesiology, perfusion, and echocardiography.

Jeffrey B. Cooper, Ph.D. is Professor of Anaesthesia at Harvard Medical School and Massachusetts General Hospital. He is the founder, Executive Director Emeritus and Senior Fellow of the Center for Medical Simulation in Boston.
Dr. Cooper is one of the pioneers in patient safety. He did landmark research in medical errors in the 1970’s and is a co-founder of the Anesthesia Patient Safety Foundation (APSF). He was for many years Director of Biomedical Engineering at the Massachusetts General Hospital and then Partners Healthcare System. He is author or co-author of over 150 peer reviewed articles and book chapters.
Dr. Cooper has been awarded several honors for his work in patient safety, including the John M. Eisenberg Award for Lifetime Achievement in Patient Safety from the National Quality Forum and the Joint Commission and the Lifetime Achievement Award from the American Academy of Clinical Engineering. He received the Distinguished Service Award of the American Society of Anesthesiologists in 2013, the only non-MD to receive the honor. He is one of the first two members of the Hall of Fame of the American College of Clinical Engineering and is among the inaugural fellows of the Academy of the Society for Simulation in Healthcare. In his personal life, he is an equestrian, avid Argentinian tango dancer and dabbles in haiku and poetry.

Karen B. Domino, MD, MPH is Professor in the Department of Anesthesiology and Pain Medicine and Adjunct Professor of Neurologic Surgery at the University of Washington, School of Medicine in Seattle. For over 20 years, Dr. Domino has been director of the Anesthesia Closed Claims Project, funded by the Anesthesia Quality Institute, the quality and safety component of the American Society of Anesthesiologists (ASA). The Anesthesia Closed Claims Project investigates adverse outcomes of patients undergoing perioperative care and acute/chronic pain management by analysis of closed claim files at participating U.S. malpractice insurance organizations. Dr. Domino is also Chair of the ASA Committee on Practice Parameters, which produces guidelines and advisories for anesthesia care.

Professor Marzida received her medical degree from the National University of Malaysia, Malaysia. In 1995, She received a Masters in Anaesthesiology from the University of Malaya. She received training in cardiac anaesthesia in National Heart Institute in 1997. She undertook fellowship in pain management at the Royal North Shore Hospital, Sydney, Australia. She has been practicing as a cardiac anaesthetist and pain specialist in University of Malaya.
Currently she is the President of Malaysian College of Anaesthesiologists and Convenor for the Pain Special Interest Group. She is in the Executive Committee of the Malaysian Association for the Study of Pain (MASP) and Treasurer for Malaysian Society of Interventional Pain Practitioners. She is board member of AARS-WFSA for 2022-2026.
Professor Mansor’s main fields of interest are in acute, chronic and cancer pain management, Cardiothoracic anaesthesia, Critical Care and Clinical Pharmacology. Her main research area is on the role of immunomodulators for the treatment of sepsis and pain. She has been an invited speaker at many national and international conferences and has published more than 30 Scientific Publications in peer-reviewed journals and books.

May Pian-Smith, MD, MS is Associate Professor of Anesthesia at Harvard Medical School (HMS) and Distinguished Scholar of Quality & Safety in the Dept of Anesthesia, Critical Care & Pain Medicine at Massachusetts General Hospital (MGH). The Chief of the Division of Quality & Safety, she oversees the coordinated work of the Quality Assurance Committee and the Quality & Safety Improvement Committee. She also oversees the Peer Support program, as supporting worker wellness and resiliency is a patient safety strategy.
Dr. Pian-Smith’s research, funded by the Foundation for Anesthesia Education & Research (FAER) and the Anesthesia Patient Safety Foundation (APSF) has focused on “speaking up” across hierarchical gradients, with the goal of improving education, enhancing the role and responsibility of all members of care teams, and supporting safety culture. She is PI of an APSF-supported study of “trust” between surgeons and anesthesiologists. She has advanced fellowship training in medical education (HMS) and patient safety leadership (NPSF-AHA) and lectures internationally about anesthesiology, medical education, safety culture and patient safety.
Dr. Pian-Smith serves on the Board of Directors of FAER and the Anesthesia Foundation. She is the President-Elect of the Society for Obstetric Anesthesia and Perinatology (SOAP), and serves on the Board of Trustees of the MGPO of MGH.

Dr. Meghan Lane-Fall is a perioperative implementation scientist and practicing anesthesiologist and intensive care physician. At the University of Pennsylvania, she is the Executive Director of the Penn Implementation Science Center at the Leonard Davis Institute of Health Economics. Her research focuses on the application of implementation science to improve patient safety and communication in perioperative and critical care settings. She serves as the David E. Longnecker Associate Professor and Vice Chair of Inclusion, Diversity, and Equity in the Department of Anesthesiology and Critical Care, with a secondary faculty appointment in epidemiology. Dr. Lane-Fall is also Vice President of the Anesthesia Patient Safety Foundation and is on the Board of Directors of the Foundation for Anesthesia Education and Research.

Meghann M. Fitzgerald, MD is the Chief of Cardiothoracic Anesthesiology at Weill Cornell Anesthesiology with expertise in anesthesia for major aortic surgery, robotic cardiac surgery and transesophageal echocardiography. Dr. Fitzgerald grew up in Buffalo, New York where she completed her bachelor’s degree at Canisius College and Doctor of Medicine at the State University of New York at Buffalo, School of Medicine and Biomedical Sciences. She completed her residency in Anesthesiology at NewYork-Presbyerian- Weill Cornell Medical Center where she served as Chief Resident. Subsequently she completed additonal fellowship training in Adult Cardiothoracic Anesthesiology at NewYork-Presbyterian-Columbia University Medical Center. Dr. Fitzgerald joined the Weill Cornell faculty in 2010. Before being promoted to Division Chief in 2018, Dr. Fitzgerald’s focus was education, developing materials and courses for students, residents, fellows and faculty on the local, regional and national and international levels. In her role as Division Chief, Dr Fitzgerald leads a team of 17 Cardiac Anesthesiologists. Under her leadership, the Division established a Consult Service for Cardiac Anestheisa, an Algorithm for Coagulation Management in Cardiac Anesthesia, and facilitated the intiation of a Chronic Pulmonary Thromboendarterectomy and re-initiation of the Robotic Cardiac Surgery Programs. Dr. Fitzgerald supports the growth of the Division, including 8 new faculty in 3 years, with clinical and academic mentorship. She also supports the Division’s mid- and senior- level faculty with advocacy and assistance with resource allocation to further the Division’s academic mission. Dr. Fitzgerald’s academic interests include resident education and Patient Blood Management. She is currently studying coagulation patterns in open thoracoabdominal aortic aneurysm repairs. Dr. Fitzgerald lives in Brooklyn with her wife and daughter.

Michael W. Champeau, M.D., FASA, adjunct clinical professor of anesthesiology, perioperative and pain medicine at Stanford University in Palo Alto, California, and president of Associated Anesthesiologists Medical Group, is president of the American Society of Anesthesiologists (ASA). He was elected for a one-year term in October 2022.
Dr. Champeau is a member of ASA’s Executive Committee and Administrative Council. He has served ASA in numerous roles, including ASA first vice president, treasurer and assistant treasurer. He has also served on ASA’s Committee on Government Affairs, the Committee on Practice Management and the PAC Executive Board.
In addition to his commitments to ASA, Dr. Champeau is past president of the California Society of Anesthesiologists and was president of the medical staff at Lucile Packard Children’s Hospital Stanford in Palo Alto, California. He is a member of the California Medical Association and American Medical Association, as well as a fellow of the American Academy of Pediatrics. He is also a recipient of the California Society of Anesthesiologists’ Distinguished Service Award. In addition, he was awarded the Lucile Packard Children’s Hospital Stanford Distinguished Medical Staff Service Award.
Dr. Champeau received his Bachelor of Science degree, with highest honors, from the University of Notre Dame in Indiana before receiving his medical degree from Yale University in New Haven, Connecticut. He completed a residency in pediatrics at the University of California, Los Angeles, and a residency in anesthesiology at Stanford. He is board-certified in anesthesiology and pediatrics.

He has published extensively in these areas and is currently funded by the NIH, NSF and foundation grants. He recently served as one of the organizers of the Stoelting Conference focused on Non-operating room anesthesia and serves as the co-Chair of the APSF Grant Alumni Academy (AGAA). He is also the Chair of the ASA abstract subcommittee on patient safety and practice management.

Dr Samuel Ern Hung Tsan is a clinical lecturer and consultant anaesthesiologist at the University of Malaysia Sarawak. He completed his Master of Anaesthesiology with Distinction from University of Malaya and is also a Fellow with the College of Anaesthesiologists of Ireland. After completing his training in 2020, he worked in Sarawak General Hospital as a consultant anaesthesiologist providing both anaesthetic and intensive care services. Currently, he is completing his neuroanaesthesia and neurocritical care fellowship at St George’s University Hospitals NHS Foundation Trust in London. His research interest lies in neuroanaesthesia and neurocritical care, airway management, and the mental health of medical professionals. He currently serves as a committee member in the Malaysian Society of Neuroanaesthesiology and Neurocritical Care.

Steven Greenberg has been practicing Anesthesiology and Critical Care Medicine at North Shore University HealthSystem in Evanston, Illinois, since 2006. He currently serves as Clinical Professor in the Department of Anesthesia and Critical Care at the University of Chicago Pritzker School of Medicine. In addition, Dr. Greenberg was recently appointed to the Jeffery S. Vender, MD, Chair of Anesthesiology Research and Education in the Department of Anesthesiology, Critical Care and Perioperative Pain Medicine at NorthShore University HealthSystem. He also served as Director of Critical Care Services at Evanston Hospital, Evanston, IL for over 12 years. Dr. Greenberg created and developed the NorthShore University HeatlhSystem Extracorporeal Membrane Oxygenation (ECMO) Program and he served as ECMO Medical Director for 6 years. The ECMO program outcomes are well above the national and international averages.
Dr. Greenberg has given over 60 lectures nationally and internationally, served on three editorial boards for anesthesia journals and published several articles and has contributed to a variety of book chapters in Anesthesiology and Critical Care Medicine texts. He has served on a variety of notable national committees including the Program Committee for the Society of Critical Care Medicine (SCCM), the Adult Concepts of Critical Care Committee for the SCCM (served as chair of the Adult Concepts course in 2014-2015), the SCCM Strategic Planning Committee, the Critical Care Steering Committee for the American College of Chest Physicians and the American Society of Anesthesiologists Committee on Critical Care Medicine. He was the Co-Chair of the 2019 SCCM Annual Congress in San Diego, CA.
With his interest in safety, he has also served on both the editorial and executive boards of the Anesthesia Patient Safety Foundation (APSF) and was appointed by the President to the position of Editor-in-Chief for the APSF Newsletter in 2017, the largest distributed piece of safety information for anesthesia professionals in the United States. He created an international editorial board for the Anesthesia Patient Safety Foundation (APSF), the first patient safety organization in the US. As Editor-in-Chief, he recruited a diverse editorial group which has brought the APSF Newsletter from 100,000 readers in 2018 to over 1,000,000 today. In 2020, Dr. Greenberg was elected Secretary of this esteemed organization. With his focus on safety, he has provided lectures for multiple national and international organizations on handoff communication, goal directed therapy, neuromuscular blockade, patient deterioration, and developing practical point of care protocols for massive transfusion/hemorrhage.

Dato’ Dr Yong Chow Yen is a consultant cardiothoracic anaesthesiologist currently attached to Hospital Lam Wah Ee, Penang. She was preciously the Head of Department of the Department of Intensive Care, Hospital Pulau Pinang. She earned her medical degree (M.D.) in University of Sciences Malaysia in 1991 followed by post-graduate training in Anaesthesiology and Critical care in University Malaya (MMed. Anaes) and United Kingdom (FRCA) from 1994-98. She trained in the subspecialty of Cardiothoracic Anaesthesiology, Clinical Perfusion and Transesophageal Echocardiograph in Penang Hospital and the Royal Prince Alfred Hospital and Royal Alexandria Children’s Hospital in Sydney Australia from 2000-2003. She served the Ministry of Health Malaysia from 1991-2022, during which she held various portfolios, notably
- Chair of Anaesthesiology and Critical Care Parallel Pathway Training in Anaesthesiology of Ministry of Health. Malaysia.
- Program Coordinator and examiner, Advance Diploma in Cardiovascular Healthcare (Perfusion), Ministry of Health, Malaysia.
- Deputy Program Head, Cardiothoracic Anaesthesiology and Perfusion.
